Primary Care Provider (MD/DO, NP, or PA)
We are supporting the Department of Veterans Affairs at the Montevideo Community Based Outpatient Clinic (CBOC) and are seeking a Primary Care Provider to deliver outpatient care in a Patient Aligned Care Team (PACT) environment. This is a stable, weekday-only role focused on continuity of care for Veterans — no nights, no weekends, no call.
Montevideo, MN (On-site)
· Provide outpatient primary care services to Veterans in accordance with VA clinical standards
· Function as part of a PACT (team-based, patient-centered care model)
· Conduct scheduled patient appointments (average ~12 patients/day; 30–60 minute visits)
· Assess and treat walk-in patients as required
· Deliver preventive care, acute care, and chronic disease management
· Diagnose, evaluate, and manage common medical conditions
· Order and interpret diagnostic tests and labs
· Document all care in the VA Electronic Health Record (EHR/CPRS)
· Participate in required quality improvement and administrative activities
· Comply with VA policies related to patient safety, privacy, and clinical documentation
Workload & Schedule Details
· Clinic Hours: Monday–Friday, 8:00 AM – 4:30 PM
· Hours: 80 hours per pay period
· Additional Time: Up to 5 hours/week for approved admin or training activities
· Holidays: Federal holidays observed (no routine coverage required)
