Primary Care Provider - Physician / Nurse Practitioner / Physician Assistant
Onsite at Dayton VA Medical Center
We are seeking Primary Care Providers to support the Dayton VA Medical Center in an onsite primary care setting. This role supports the VA PACT primary care model and may be filled by qualified Physicians, Nurse Practitioners, or Physician Assistants
Position Responsibilities
· Provide onsite primary care evaluation and treatment for veteran patients.
· Order and interpret labs, diagnostic imaging, EKGs, and medications.
· Coordinate consultations with subspecialists.
· Support discharge and follow-up planning.
· Conduct end-of-life and advance care planning discussions.
· Perform office-based procedures within scope, including items such as abscess incision and drainage and fecal occult blood testing.
· Document care in the VA electronic health record by the end of each encounter.
Required Qualifications
· MD, DO, Nurse Practitioner, or Physician Assistant credentials, as applicable.
· Current license in good standing, as applicable.
· Physicians must hold a current, full, and unrestricted license in any U.S. state, territory, commonwealth, or the District of Columbia.
· Physicians must be Board Certified or Board Eligible in Internal Medicine.
· Current BLS and ACLS or equivalent.
· Current DEA registration.
· NPI number.
· Ability to obtain and maintain VA credentialing and privileging before start.
· Ability to speak, read, write, and understand English fluently.
Eligibility Requirements
· U.S. Citizen; or
· Permanent Resident/Green Card holder with a Social Security Number and must have resided within the United States for the past three years;
