Licensed Practical Nurse (LPN) - Primary Care

We are seeking Licensed Practical Nurses to support onsite Primary Care PACT services at the Dayton VA Medical Center. This role supports patient care delivery, documentation, team-based operations, and patient flow.
Position Responsibilities
· Provide direct and indirect nursing support in the primary care environment.
· Obtain patient history and support assessment and care planning.
· Document care and report changes in condition to the RN or physician.
· Monitor patient response to medications and therapies.
· Assist in emergencies and support diagnostic or procedural care.
· Review labs and medical records as part of patient support workflow.
· Provide patient and family instruction.
· Participate in discharge planning and team-based care functions.
· Maintain professional conduct, patient privacy, and customer service standards.
Required Qualifications
· Graduate of an accredited practical nursing program.
· Current Ohio LPN license.
· Current CPR certification from an American Heart Association vendor.
· Current BLS certification.
· Minimum one year of direct patient care experience within the last three years.
· VA experience preferred, not required.
· Ability to obtain and maintain VA credentialing and clearance requirements.
Eligibility Requirements
· U.S. Citizen; or
· Permanent Resident/Green Card holder with a Social Security Number and must have resided within the United States for the past three years;
· No sponsorship available.
